An opportunity has arisen with Chelmsford West PCN for a part-time (up to 24 hours a week) Care Co-Ordinator to support our team with the day-to-day administration of care home services, providing support to our Safeguarding team, and supporting our patients who have cancer.
The successful applicant will have the ability to work on your own initiative as well as within a team, have good computer skills and the ability to work under pressure.
Previous experience of working in primary care and knowledge of SystmOne would be desirable but not essential as full training will be provided.
Chelmsford West PCN represents Chelmer Medical Partnership and Writtle Surgery. The role will be based at Chelmer Medical Partnership which has three sites Humber Road Surgery, Melbourne House Surgery and Tennyson House Surgery. Candidates will be required to work across these three sites.
