Care Co-ordinator/Administrator (PCN INT)

As part of an exciting service expansion, Chelmer PCN is recruiting a Care Co-ordinator/Administrator to join our Integrated Neighbourhood Team (INT). This role supports patients with complex needs across general practice, community services, and social care — ensuring they receive personalised, coordinated care.

Chelmer PCN is a forward-thinking Primary Care Network committed to improving patient outcomes through integrated, person-centred care. This role is part of our growing INT team, designed to deliver proactive support to our most vulnerable patients.

Main Duties of the Job:

• Work closely with GPs, nurses, social workers, and voluntary sector partners
• Support patients with long-term conditions, frailty, or social vulnerabilities
• Facilitate personalised care planning and proactive case management
• Help patients access community support, social prescribing, and wellbeing services
• Maintain accurate records and contribute to service development

Person Specification:

• Experience in health, social care, or community support
• Excellent communication and organisational skills
• Ability to work independently and collaboratively
• Understanding of integrated care and personalised support
• IT literacy and willingness to learn clinical systems
