The Practice Nurse will work as part of the primary care team to deliver a high standard of nursing care to patients registered with the practice. This includes the management of long-term conditions, health promotion, screening, immunisations, and treatment of minor illnesses within the scope of professional competence.

The role requires excellent clinical skills, effective communication, and a commitment to patient-centred care in a busy general practice environment.

Key Responsibilities

Clinical Duties

  • Provide nursing care to patients in accordance with practice protocols and professional standards.
  • Manage and support patients with long-term conditions such as asthma, COPD, diabetes, hypertension, and coronary heart disease.
  • Undertake cervical cytology, childhood and adult immunisations, travel vaccinations, and wound care.
  • Perform ECGs, blood pressure checks, and other diagnostic or monitoring tests as required.
  • Provide health promotion and lifestyle advice, including smoking cessation, weight management, and sexual health.
  • Support chronic disease management clinics and review patients’ care plans.
  • Administer and monitor medication under patient group directions (PGDs).
  • Maintain infection control standards and contribute to audits and health & safety processes.
  • Administrative and Professional Responsibilities
  • Maintain accurate and up-to-date patient records using the practice’s clinical system (e.g. SystmOne).
  • Participate in clinical audits, significant event reviews, and quality improvement initiatives.
  • Contribute to the achievement of QOF (Quality and Outcomes Framework) and Enhanced Service targets.
  • Ensure compliance with CQC (Care Quality Commission) standards.
  • Attend practice meetings, training sessions, and annual appraisals.
  • Maintain registration with the NMC and adhere to the NMC Code of Professional Conduct.
  • Essential Qualifications and Experience
  • Registered Nurse (RGN) with current NMC registration.
  • Experience in primary care or community nursing.
  • Up-to-date training in immunisations, cervical screening, and infection control.
  • Evidence of continuing professional development.

Desirable Skills and Experience

  • Experience in chronic disease management (e.g., diabetes, respiratory care).
  • Knowledge of QOF, clinical governance, and CQC requirements.
  • Proficiency in using clinical IT systems (SystmOne).
  • Independent prescribing qualification (V300) – desirable but not essential.
