Working within a GP setting, you will empower patients to improve their wellbeing by connecting them with local, non-clinical support services and community groups. You will adopt a holistic approach, helping individuals set personal goals.
Key Responsibilities
- Caseload Management: Provide 1-to-1, personalized support to co-produce care plans.
- Clinics: Conduct in-person or virtual sessions for patients referred by the clinical team.
- Community Asset Building: Strengthen links with local organizations and identify service gaps.
- Collaborative Working: Partner with GPs and other healthcare professionals to support patient needs.
- Help with QOF related work: Spreadsheets.
