We are looking to recruit an ACP/Paramedic who will provide a high-quality acute home-visiting service for patients requiring urgent same-day clinical assessment within the community.
The role focuses on preventing unnecessary hospital admissions, supporting frail and vulnerable patients, and delivering timely, autonomous clinical decision-making.
The postholder must be a qualified Non-Medical Prescriber (NMP) and capable of managing a wide range of acute presentations seen in primary care.
