We are seeking a highly motivated, enthusiastic and experienced Advanced Nurse Practitioner to provide support to our loyal practice team.

Longfield Medical Centre is a forward thinking, well established, friendly training practice with six GP partners and a list size of just under 17,000 patients. We are focused on providing the highest standard of care to our patients and are supported by a large clinical and administrative team.

The post-holder will be an experienced nurses who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

Duties will include:

  • Assessing, diagnosing, planning, implementing and evaluating treatment and interventions, as well as caring for patients presenting with an undifferentiated diagnosis
  • Clinically examining and assessing patient needs from a physiological and psychological perspective, and planning clinical care accordingly
  • Assessing, diagnosing, planning, implementing and evaluating interventions/treatments for patients with complex needs
  • Proactively identifying, diagnosing and managing treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Diagnosing and managing both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
  • Prescribing and reviewing medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.

About the Candidate – The successful candidate will:

  • Be a registered first level nurse, MSc or equivalent, holding current registration with the NMC
  • Hold an independent prescriber qualification
  • Hold a mentor/teaching qualification
  • Have clinical supervision training
  • Have advanced clinical practice skills
  • Be able to work with a high degree of autonomy
  • Be confident in leading in a variety of settings: your leadership of others will also enable you to use and further develop your teaching and mentoring experience/qualifications
  • Be forward-thinking and will strive to develop innovative ideas and new ways of working, that address the health needs of the practice, whilst motivating and encouraging others to do the same
  • Be an excellent communicator and collaborator
  • Embrace opportunities to work and liaise with others, across multi-professional and multi-agency boundaries, in the interests of improving patient outcomes
  • Be consistently compassionate and respectful, placing a high value on the dignity of patients, relatives, carers and colleagues.
