We are looking to welcome an enthusiastic part-time Diabetes Practice Nurse to our well-established surgery to run the diabetic clinic. You will be supported by clinical and admin teams.
The main responsibilities of the role include:
- To act as a clinical expert for diabetic care.
- Work in conjunction with other disciplines in the assessment, planning, implementation and evaluation of Services provision ensuring a smooth pathway for patients with diabetes.
- Responsible for providing clinical leadership and education.
- To represent the practice at Diabetes meetings when necessary.
- Manage their time appropriately according to patient and service needs.
- Responsible for the promotion and use of the audit cycle within clinical practice.
- Support the development and implementation of nursing services, policies and procedures.
- Liaise with primary and secondary colleagues, in delivering and supporting effective specialist diabetes care.
You will have the following skills and qualifications:
- A Registered Nurse
- Qualified Nurse Prescriber
- Diabetes module or qualification achieved at degree or Master level
- Experience of chronic disease management and undertaking medication reviews
- Compassionate, empathetic approach and demonstrable kindness in all aspects of your work as well as valuing the dignity of our patients, relatives, carers and colleagues.
- Knowledge and application of supporting long-term conditions.
- Insight and understanding of current issues in nursing and NHS.
About Us
Our Surgery comprises of around 8300 patients. We operate a SystmOne clinical system. We have a history of high QOF outcome for diabetes.
