Details of Role
We’re on the lookout for a capable and confident Practice Nurse to join our primary care team. This is a part-time role (28 hours per week). You must be experienced in the full scope of general practice nursing duties.
The main responsibilities of the role include:
• Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
• Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met.
• General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
• Managing and treating long-term conditions, in line with national and local policies and the practice needs.
• Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
• Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
• Supporting and managing the health needs of women presenting for family planning and cervical cytology consultations.
• Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
• Meeting the needs of patients presenting for opportunistic wound care.
• Hypertension review and management.
• Running asthma, COPD and diabetes clinics and reviews.
• New patient and health checks.
• Travel health advice and vaccinations.
• Infection control & health promotion.
• Support for QOF and enhanced services.
What We’re Looking For:
• Registered Nurse with current NMC registration.
• Prescribing qualification would be preferred.
• Proven experience in a general practice or primary care setting.
• Up-to-date with current protocols, PGDs, and best practices.
• A team player with great communication skills.
• Someone who takes initiative and doesn’t need their hand held.
• Someone who is flexible, reliable, and professional.
