Care Navigator

We are looking for an enthusiastic and hardworking Care Navigator to join our busy GP practice.

You will be the first point of contact either face to face or on the telephone and must work well as a team member.

You will need excellent communication skills and be able to work under pressure.

The successful candidate will also be required to undertake additional shifts to cover colleagues annual/sick leave.

Working knowledge of EMIS Web would be an advantage but not essential as full training would be provided.
