Working as part of Nuffield House you will assist in connecting people to appropriate services and opportunities that can meet their support needs.

This role involves a wide range of responsibilities including:

  • Liaise with people registered at the practice, their families/representatives, healthcare professionals, social services and GPs to ensure that their care & support needs are being met.
  • Assess, plan, develop, implement, and evaluate non-clinical support that promote health & wellbeing.
  • Work with patients to support adherence to prescribed clinical treatments.
