We are recruiting an Advanced Nurse Practitioner on a part-time basis.

The role will have responsible for consulting with patients either face-to-face or by telephone; diagnosing and treating patients presenting with minor illness and advising people in respect of their continuing medical and nursing needs.  You will also consult with patients regarding their disease management and other ad hoc Practice Nurse tasks. There is a requirement to see extra or emergency patients.

The nurse practitioner will need to prioritise and triage the needs of patients making any necessary referrals to the duty or registered doctor as appropriate. Nursing home visits and assistance of home visits and vaccination programmes will be required. Annual review of patients within the disease management of QOF

The post-holder will:

  • Apply Practice policies, standards and guidance
  • Discuss with other members of the team how the policies, standards and guidelines will affect own work
  • Participate in audit where appropriate
