We are looking to recruit a friendly, enthusiastic, highly motivated part-time (20 hours per week) clinical Pharmacist to join our growing, dynamic four Partner Training Practice in Basildon:

The post holder will work within clinical competencies as part of a multidisciplinary team, to provide expertise in medicines management as follows:
Provide structured medication reviews.
Manage long term conditions, medicines on transfer of care and systems for safer prescribing, repeat authorisation and acute prescription requests.
Ensure that medicines management processes within the Practice are optimised to achieve highly effective, safe and patient centred prescribing.
