Practice Based Care Coordinator

Little Waltham & Great Notley surgery are looking for a full time practice-based patient Care Coordinator to join our team.

As a patient Care Coordinator you would be required to deal with patients and their carers, working closely alongside our multidisciplinary team.

Being practice-based you would be able to identify patients’ individual needs ensuring they are addressed by taking into account local priorities, health inequalities and population health management risk stratification.

Effective communication skills and a compassionate caring approach is essential for this post.
